Vet consent form

How to use this form:

1. Click the link below to download the form.

2. Email the form across to your vet and ask them to sign it or print it off and take it into the practice and ask for it to be signed.

3. If they are happy for your dog to receive massage treatment they will return the form to you, signed.

4. Email the form along to me in time for you dog's first session or print it off and give it to me at their first session.

Remember I cannot massage without a signed vet consent form. If you have any issues please get in touch and I will help in any way I can.
